Description: Child and Adolescent In-Home Case Manager
Salary: $17.31 hr
Qualifications: Bachelor degree in social work, psychology or mental health-related field required.
Preferred Experience: Knowledge of community mental health. Field/Crisis work with children and adolescents.
Duties and Responsibilities: Provide mental health co-therapy/case management services to child and adolescents and their families in their home or community setting as a member of a two-person team paired with a master-level therapist. Complete needs assessments and develop case plans. Works closely with the family unit and community programs for the welfare of the child/adolescent. Works to keeping individuals stabilized in the community settings and assists in securing psychiatric hospital placement as needed.
